Please see below the new Patient registration form that can be printed and filled ahead of time before visiting our office for your appointment. This will save you time when you visit our office for the first time as a new patient.

During COVID-19 Quarantine we are limiting our Visits to Televisits only. Please read guidelines below if you are a new patient. If you are an existing patient you don’t need to fill new patient forms again.
What types of appointments can be done through Televisit?
During COVID-19 Quarantine you can be seen for: Cold, UTI, Fever, Diabetes, High BP, Allergies, Sick Visits, Medicine Refills, Asthma, Gynaecology Issues and other Health Problems. Please call our office to check if a Televisit will be possible for your coniditon!
Can insurance be used for Televisits?
Yes! We will bill your insurance for the visit. If you do not have insurance, there is a $75 fee that must be paid prior to the start of your Televisit appointment.Type your paragraph here.
New Patients?
Yes we are accepting New Patients for Televisits who have Insurance as well as those who don’t have Insurance\and are self pay. For new patients we want all our patients to fill the New patiet Child Form for the child Televisit.
